Health Needs Addressed: Access & Delivery to Health Care

Funding from Stanford Health Care supports the establishment of a new Social Work Program at Ravenswood Family Health Network. The overall goal of the Social Work Program will be to address the social determinants of health (SDOH) needs of patients, especially high-risk, complex patients, who are most often underrepresented minorities. The program will provide concentrated support to patients and their families to address social, financial, and psychosocial needs related to their health conditions as they move through the health continuum.
